Thank for your interest and decision to pursue treatment for better health. The first thing that Dr. Farber needs is for you to fill out some forms and send them back to us. You will do this via email by following the instructions below.

We suggest that you first create a folder on your desktop called Dr. Farber and then return to this page.

• Right-click on each of the forms below and select Save Target As .... It will already have a file name so all you have to do is navigate to your new folder and click save.
• Do this for each of the forms.
• Fill out each of the forms, using your tab key to move through the form and save them back to your folder.

Once this is done, click here and you will be taken to a web site where you can attach the forms and send them to Dr. Farber.

• The To: field will be lance@lancefarber.com
• The From: field will be your email address.
• You can enter some comments in the text box if you wish.
• Then click on Attachments and you will be taken to a page where you can browse to your folder and select each of the forms.
• After all of them have been selected, click Attach and you will be returned to the previous form (This process may take a few minutes, so please be patient).
• Once the transfer is complete, click send and you're done
